Quotation Process

  • STEP 1

    Select "Order Request" in the "Details of Request" section of the form and fill in the required information.

  • STEP 2

    After submitting the form, you will receive a confirmation email.

  • STEP 3

    We will send you a quotation to the e-mail address you entered in the form within a week.

Ordering Process

  • STEP 1

    Select "Order Request" in the "Details of Request" section of the form and fill in the required information.

  • STEP 2

    After submitting the form, you will receive a confirmation email.

  • STEP 3

    We will send you a quotation and invoice to the e-mail address you entered in the form within one week.

  • STEP 4

    Please check the contents of the quotation, and if you are satisfied with the quotation, please make a bank transfer to the bank account indicated on the invoice.

  • STEP 5

    We will arrange for the manufacturing of the product and inform you of the delivery date.

  • STEP 6

    After receiving the product, please inspect the product and check its operation.

Notes on sending case data

In case of file upload

  • (1)Please prepare an STL-format file converted from DICOM data of a 3D DSA image capturing the affected blood vessels (voxel size recommended to be 0.15 mm or smaller) using a medical imaging workstation, or an open-source software like 3D Slicer. If you have any questions regarding data preparation, please contact us via the "CONTACT" page on our website.
  • (2)Please ensure that all data is anonymized.
  • (3)As a general guideline, the model to be created should fit within dimensions of 100mm × 100mm × 70mm (length × width × height), including the affected area.
  • (4)Please specify the type of device intended for use and provide information that identifies the affected area in the designated fields.
  • (5)Please ensure that the STL data is a completely closed 3D model before submitting.
  • (6)Please compress the STL data file in ZIP format. The maximum file size allowed is 200MB.

Use of Personal Information for Direct Orders and Quotations of Case Models

In case models, actual patient diagnostic data is used. Medical specialists convert medical imaging information (DICOM) obtained through CT, MRI, or MRA into 3D data (STL) of vascular shapes. Blue Practice then uses 3D printing technology to manufacture the models. Orders and quotation requests must be submitted along with the desired case data. Once the vascular model (case model) is created, it will be delivered directly to the specified location.


Collected Personal Information

The following personal information is required for ordering, estimating, and manufacturing case models:
• Request Information: Order request, quotation request
• Customer Information: Hospital/facility name and department, name, furigana, email address, phone number, postal code, prefecture, city, street address, building name, etc.
• Medical Information: STL data, specified lesion site, date of receipt


Purpose of Use of Personal Information

Customer and case information will be used for the following purposes:
• To confirm request details (via phone, email)
• To send quotations and invoices (via phone, email, or mail)
• To communicate regarding manufacturing (via phone, email)
• To manufacture the case model
• To deliver products and delivery slips

Outsourcing of Personal Information Handling

This website uses data centers for form handling and Japan Post and Yamato Transport for shipping products. Part of the manufacturing process may be outsourced.


Provision of Personal Information to Third Parties

Customer personal information will not be provided to third parties.


Correction and Suspension of Use of Personal Information

To confirm, correct, or suspend the use of registered or case information after submitting an order or quotation, please contact your sales representative or the Personal Information Inquiry Desk.
